Enclosures:

1. SBI E-Challan original (University copy) towards Registration of Application.
2. Attested Xerox copies of all marks statements of Final MBBS & Post Graduation
3. Attested copy of Medical Registration Certificate of Post Graduate Degree from the respective State Medical Council.
4. Attested copy of Provisional or Original MBBS and Post Graduation Degree Certificates.
5. Attested copy of conduct and character certificate from the college in which last studied.
6. Attested copy of study certificate of PG & MBBS courses.
7. If the applicant has not studied Post Graduation in the State, shall submit the relevant certificate as per regulation 3.8 whichever is applicable to the applicant.
8. If the candidate passed Post Graduation from State wide institutions i.e., SMC, Vijayawada, NIMS and SVIMS etc., he / she have to submit Study Certificate of MBBS.
9. If the candidate passed Post Graduation and MBBS in the State wide institutions i.e., Siddhartha Medical College, Vijayawada and SVIMS and NIMS, he / she has to submit Study Certificate from 6th class to Intermediate in addition to MBBS and PG Study certificates.
10. If a Candidate discontinued M.Ch / DM Courses, Discontinuance Certificate as in Annexure - II issued by the Principal concerned, should be enclosed.

Candidate must be pass in MD in a relevant discipline from any of the Universities incorporated by an act of the central or state legislature in India.
