Siddhartha Medical College Admissions 2024: Dates, Courses, Eligibility, Selection Process

The application form for SMC will be available online. Candidates can apply for Siddhartha Medical College after getting NEET Scores. To apply for NEET, candidates must follow the steps below:

• Visit the official website and click on the application form button and register with a valid unique email ID and phone number. 
• Fill the application form with personal and educational details.
• Upload scanned copies of a passport size photograph, a post size photograph, their signature, a left-hand thumb impression, as well as a class X passing certificate.
• Post the filling of the application form, download the system generated application number. 

Documents required at the time of admission

• Admit Card
• Scorecard or Rank Letter
• Class 10 certificate and mark sheet (for date of birth)
• Class 12 certificate and mark sheet
• ID proof (Aadhar/PAN Card/Driving License/Passport)
• Original Copies Of (in case of MS and MD):

1. MBBS Certificate.
2. MBBS 3rd Part II mark sheet.
3. Medical Registration Certificate.
4. Internship Completion Certificate.

• Photo identity proof in the form of Certificate with an attested photo from Head of Institution last studied/Copy of Passport/Aadhar Card/PAN Card/Driving License (Self-attested photocopy after verifying with the original documents).
• Copy of: Birth Certificate for proof of age (if the date of birth not given in the HSC mark sheet/ certificate).
• NEET Score Card.
• Copy of Online Counselling Registration Certificate.
• Sponsorship Certificate from State Govt. If applicable.
• Passport size Photographs 8 copies.
